Financial Arrangements

Evergreen Surgical Center is committed to reducing the cost of quality healthcare. To simplify your bill, we have established a single basic charge for each procedure. This covers your use of the facility, including pre-operative evaluation, operating and recovery rooms, equipment, supplies, and drugs used while at the Center. A Patient Account Representative from Evergreen Surgical Center will call you to discuss the amount required for your particular surgery and insurance plan. Should your insurance not cover the full cost of your stay, a deposit will be required on the day of surgery.

For your convenience, cash, personal check, Visa, Discovery, or Mastercard are accepted.

The basic charge does not include the following:

Surgeon’s fee or the fees of the anesthesiologist, pathologist, radiologist, and/or other physician
Prescriptions for home use
Blood and blood substitutes
Laboratory work
Expenses associated with transfer to a hospital

You will receive separate bills for the above services.

Please be sure to bring your insurance card, insurance coupon, and your photo ID.
